Collection Instructions:
SST Collection: Allow specimen to completely clot (this can take up to 60 minutes). Centrifuge for 15 minutes. The serum should be physically separated from contact with cells within 2 hours from the time of collection. The tube must be spun down and serum aliquoted into a transfer tube prior to freezing and transporting to the lab. 
EDTA/Lt Green Collection: Centrifuge and transfer plasma into a properly labeled transfer tube within 2 hours of collection.

Clinical Significance:
PTH measurements are used in the differential diagnosis of hyperparathyroidism, hypoparathyroidism, or hypercalcemia of malignancy. Assay results should be used in conjunction with other clinical data to assist the clinician in making individual patient management decisions. Parathyroid hormone (PTH) is secreted by the parathyroid gland. PTH plays a crucial role in maintaining calcium homeostasis and its measurement is an important aid in the diagnosis of calcium related disorders. PTH regulates calcium levels by concerted effect on three principle organs: bone, intestinal mucosa, and kidney. In healthy individuals, PTH secretion responds to small alterations in plasma ionized calcium concentrations within seconds. Abnormally low ionized calcium concentrations trigger PTH secretion, whereas rising levels of extracellular calcium reduce PTH secretion through a negative feedback mechanism.
