Collection Instructions:

Tubes must be filled to completion to ensure a proper blood to anticoagulant ratio.  Mix tubes gently by inverting 5-10 times immediately after filling.  Centrifuge for 15 minutes within 1 hour of collection.  Using a plastic pipette, remove plasma taking care to avoid the WBC/platelet buffy layer and transfer plasma into a properly labeled transfer tube.  Centrifuge the transferred plasma a second time and transfer the platelet poor plasma into a new properly labeled purple frozen transport tube.  Label tube as "platelet poor".  Freeze immediately.

Limitations:

If the patient's hematocrit exceeds 55%, the volume of citrate in the collection tube must be adjusted.

Icteric (Bilirubin > 13 mg/dL) and hemolyzed (Hemoglobin > 125 mg/dL) specimens may give false results. 

Additional Information:

It is preferable for samples to be obtained prior to, or in the absence of, anticoagulant therapy as this may interfere with the assay.  Caution should be exercised in interpretation of the results of tests performed close to a thromboembolic event.

Method: Silica Clotting Time (SCT) and dRVVT screen; mixing study if screening tests are prolonged; confirmation if the mixing studies do not correct
Clinical Significance: Lupus anticoagulants are nonspecific antibodies that extend the clotting time of phospholipid-dependent clotting assays such as the aPTT.  Unlike specific factor antibodies, LA are usually associated with venous thrombosis, pulmonary embolism, arterial thrombosis, and recurrent fetal loss.
