Test Name: CREATININE CLEARANCE, 24-HOUR URINE
Test Code: 2120182
Alias: LAB818
CPT Code(s): 82575
Test Includes:

Creatinine, 24-Hour Urine

Creatinine, serum or plasma

Creatinine Clearance

Glomerular Filtration Rate

Preferred Specimen:

5 mL urine of a well-mixed, 24-hour collection and

1.0 mL serum

Container:

Amber 24-hour urine plastic container (no preservative) and

SST (gold)

Alternate Container:

24-hour urine:  Amber 24-hour urine plastic container with preservative (5 mL 6N HCl or 5 g boric acid)

Blood:  Lithium Heparin (lt green)
            EDTA (lavender)

Minimum Volume:

1.0 mL urine of a well-mixed, 24-hour collection (no preservative) and

0.3 mL serum or plasma

This volume does not allow for repeat testing.

Collection Instructions:

The collection container must be kept refrigerated or on ice during collection. Instruct patient to void at 8 AM (or 8 PM) and discard the specimen. Then collect all the urine including the final specimen voided at the end of the 24-hour collection period (ie, 8 AM or 8 PM the following day).

Refer to separate patient instructions for detailed 24-hour urine collection. Record the collection date/time, total volume and patient's height and weight on the specimen container and test requisition.

Blood sample will be collected at the end of the same 24-hour period of urine collection. Allow specimen to completely clot (this can take up to 60 minutes). Centrifuge for 15 minutes. The serum should be physically separated from contact with cells within 2 hours from the time of collection. If the specimen is collected in an anticoagulant tube, centrifuge and transfer plasma into a properly labeled transfer tube within 2 hours of collection.

For patient safety, dropping off specimens requires a full registration process and verification of proper specimen collection.

Transport Temperature: Refrigerated
Stability:

Refrigerated: 6 days (24-hour urine)

Room Temperature: 7 days (serum or plasma)

Refrigerated: 7 days (serum or plasma)

Rejection Criteria:

No blood drawn

Patient's height and weight not documented

Limitations:

Errors in recording timing of collection period

Loss of a portion of the urine during collection

Including the first void in the timed collection

Urinary retention

Additional Information:

Hydrate the patient by administering a minimum of 600 mL of water. Withhold coffee, tea and drugs on the day of the test. Keep the patient well-hydrated during the urine collection period in order to insure a urine flow rate of 1-2 mL/min or greater. The blood can be collected at any time during the urine collection period . However, blood collection at midpoint of the urine collection period is recommended. Include the patient's height (in feet and inches) and weight (in lbs.) for determining body surface area.

Schedule: Set up: Daily; Results in 24-48 hours
Method: Jaffe rate
Clinical Significance: The creatinine clearance test is used as a measure of glomerular filtration rate.
Use: Updated 2/28/22
